Group Therapy: $55 (90 minutes)
Individual Therapy: $225 (50 minutes)
Dr. Amidon keeps 20% of client slots at a sliding scale, although sliding scale availability cannot be guaranteed. If complicated reports are needed, there may be a charge for me to prepare it.

If you are a member of a PPO plan, your insurance may reimburse you directly for my services as an out-of-network provider. PPO plans typically reimburse around 70-80% for out-of-network providers. Please call your insurance and ask about their policy on out-of-network behavioral health care. You will want to know (1) the amount of your deductible and (2) what percentage of behavioral health services they will reimburse you. You will be responsible for paying for your session in full at the time of service. I can either provide you with a monthly bill to submit to your insurance company for reimbursement or I can submit directly to your insurance company as out-of-network.
